You will need to ensure this is complete if you are being paid by BACS. Please note BACS forms need to be accompanied by an original bank statement.

Reporting back to us
Twice a year you will need to fill out a report online to let us know who you allocated awards to and what they spent their money on.
These reports are due by 30th June and 30th November every year and you can report back to us up to a month in advance.
During these periods the green button opposite will 'go live'.
